Job Duties: Care coordination of the Children with Medical Handicaps (CMH) program; conducting home/ telehealth visits with families; assessing family needs and providing support, information, and referrals as necessary; completing all nursing documentation in the electronic medical records and billing as required by CMH; performing chart reviews and implementing quality improvement measures. Active role in providing infant, children, adult, and international travel vaccinations in CCPH immunization clinics on a routine basis; participating in the immunization program planning and implementation. Conducts management on all reported cases of Sudden Infant Death Syndrome (SIDS) and Childhood Lead Poisoning occurring in Clermont County. Participates in outreach activities to educate clients, providers, agencies, and the community about CCPH Community Health Service programs. Actively participates in continuous quality improvement with the agency through individual work plans and/or projects. Participates in activities to ensure and maintain accreditation status. The individual filling this position will also be trained in basic CCPH-related emergency response procedures and considered essential for any public health emergency response and disaster recovery. Minimum Qualifications: • Ohio license to practice as a Registered Nurse (Section 4723.01 of Ohio Revised Code) (Required) • Bachelor's Degree from an accredited school of nursing (Required) • Six months experience in public health nursing or equivalent nursing experience (Preferred) • Cardiopulmonary resuscitation (CPR) certification (American Heart Association or equivalent) (Preferred) • Knowledge and skill in administering vaccinations to infants, children, and adults (Preferred) • Care Coordination and home visiting experience (Preferred) • Knowledge and proficiency using an Electronic Health Record system (Preferred) • Independent worker • Skills needed in organization, networking, and basic computer skills • Must have and maintain a valid driver's license • Required to have regular and predictable attendance Fringe & Benefits: Benefits & Fringe Package Value Full-Time: $22,656 to $70, 928 Benefits & Fringe Package Value Part-Time: $7,853 to $32,773 Clermont County Public Health offers a generous fringe and benefits package to all employees: • Sign-On Bonus- New Employees receive a one-time $500 sign-on bonus after completing their 180-day probationary period. After one year of service, staff will receive an additional $1,000 bonus. • Attractive Schedule- We follow an attractive work schedule of 8:30 am to 4:30 pm, Monday through Friday. Full-time employees have the option of a four-day (10 hr./day) workweek once the probationary period is complete.
